How To Refer A Patient


Patient Rights

To refer a patient to one of scheck & siress’ 15 convenient locations:

 

Outpatient: Provide your patient with a prescription with a diagnosis and the device or service he/she requires. If you are not certain of the most appropriate device for your patient, describe the objective. For instance, “relieve ankle pain due to arthritis” or “off-load foot ulcer to facilitate healing.” We will evaluate the patient and contact you with our recommendation.

In-patient: Call us with a name, room number, prescription, or fax your order to a Scheck & Siress location near you.

Physical and Occupational Therapists: Call one of our 15 locations to speak with a clinician for a phone consultation.
